COVID-19 UPDATES

On April 6, 2023, Provincial Health Officer Dr. Bonnie Henry announced changes to the COVID-19 guidance for health care settings. This change included an end to universal masking.

  • We will continue to make masks available at reception and childcare/preschool- there will still be some situations where masks will be required to protect medically-fragile children against respiratory transmitted infections.
  • Please stay home if you are feeling unwell and wear a mask if you or a family member have any cold or flu symptoms.
  • We ask that you please continue to clean your hands throughout your visit (wash hands with soap, use hand sanitizer).

Thank you for your support, helping us protect the medically-vulnerable children we serve!

For more details about COVID-19 procedures and additional information, please see our COVID-19 Updates page.

REFERRALS

Do I need a doctor to put in a referral?
Anyone can submit a referral as long as the child’s guardian is aware. The only exception is for our Feeding & Nutrition team services- a doctor must submit a referral.
Will my referral have a higher priority if it is done by a doctor?
No. Our Intake team uses the information on the referral form to make priority decisions.
Will multiple referrals help?

No, multiple referrals won’t change the outcome but additional information can be sent to help the Intake team decide on the priority status of a referral.

Can I send in a referral for the Feeding & Nutrition Team?

Only a doctor can refer for this service.

REFERRAL FORM

Can someone help me to complete the referral form?

Yes. As long as you are aware of what is written on the form and sign the consent section on page 4, you can have help. The person helping you should be listed under “Associated Professional Assisting” even if they are not a “professional” like a doctor.

What do I put in the “The Name of the Associated Professional(s) who assisted to make the referral”?
If you are the guardian/parent and completing the referral without help, leave that section blank. If someone is helping you with the referral, their information should be listed in this section.

What do I do on page 4 – Consents?

It is helpful for you to initial by the agencies/professionals that your child is working with. This will give permission for us to communicate with them and get complete information for your child’s referral. Page 4 must also be signed for consents to be valid.

Form Sample 2

Do I fill in all the blanks on the referral?

It is important to make sure page 1 has complete information and the Consent section on page 4 is initialed and signed. When completing pages 2 and 3, only fill in detailed information for the services you are requesting. If you are not requesting a service, please leave it blank.
